Dosimetry Contract
Please find attached the dosimetry contract for your kind attention, the contract lays out the general requirements for facilities that are using Authority dosimeters as well as issues of proper handling, use and return of dosimeters. It is now a requirement for facilities that require dosimeters to sign the contract for the provision of the dosimetry service.
Please complete the form and submit it to RPAZ at your earliest convenience. You are also reminded to ensure dosimeters are submitted to RPAZ for reading as per the dosimetry schedule for your facility.
